Banned! (and why)

I thought it'd be a good idea to keep a running list of all the people who've been banned and the reasons for their being banned. This way, if somebody asks to be un-banned, then any Op/Admin can check this list and decide whether the banned person deserves to be let back in.
Only those capable of banning/unbanning should be permitted to post here. All other posts should be deleted as this is NOT a thread for debate. If you want to ask to be unbanned, then post a new thread.
